Solution

Unpack the attachment to SAP_SM.XLS to a local file. You can use the report BCALV_BDS_MAINTENANCE to import the template into your system as "SAP Standard Templates" as described in SAP Note 548409, point 3 (about importing changed templates). You can use the checkbox "Transport Request" on the selection screen to set the template to a Customizing request and distribute it in the system landscape.
